Job Title: RN Case Manager
Two locations: Chico and Placerville

Position Summary:
In this opportunity you will get to perform collaborative duties to assess, plan and coordinate continuum of care for select members at high risk in order to promote quality, cost effective programs.
Hours for this Position:
Monday – Friday 8:30-5

What We Look For in a RN Case Manager:
Active RN CA license
2+ clinical experience
2+ managed care experience
Some case management experience
Comfortable on the phone

More Insight of Daily Responsibilities as RN Case Manager:
You will review admissions through on-site and telephonic review to ensure medical necessity and appropriate level of care.
Develop, assess and adjust as necessary the care plan and promote desired outcome.
Coordinate services between Primary Care Physician (PCP) specialist, and other medical and non-medical providers as necessary to meet the complete medical socio economic needs of clients.
